  • 2023 Blank Annual Census Form
  • Amendment or Discontinuance of Business
    • File this form if your business is no longer operating, changed addresses or ownership. To change the name of your business, you will first need to discontinue it and refile under the new name. The fee to discontinue or amend your business is $20.00.
  • Business Certificate (D/B/A Form)
    • For new business filings or renewals. The filing fee is $60.00.
  • Campaign Finance M101 - Statement of Organization 
    • This form contains the names and addresses of the committee officers. Any change in information should be reported within 10 days of the change. If there is a change in treasurer, file Form CPF T101 with our office.
  • Campaign Finance Form M102 
    • Local candidates and political committees are required to file this form with our office on or before each reporting date. Candidates and treasurers are responsible for the legality, validity, completeness and accuracy of each of their reports. 
  • Campaign Finance Form M102-0 
    • This form may be filed in lieu of the M102 by local candidates who do not have a political committee organized on their behalf, and have not received any contributions, spent any money, maintained a balance or incurred any debts.

    • If you would like to request a birth, marriage or death certificate by mail, you may print this form and mail it in along with a check or money order made payable to the Town of Amherst. Certified copies are $10.00 each. Processing time is typically one business day.
  • Non Resident Clergy Application
    • Members of the clergy or Justices of the Peace from another state may officiate your wedding in Massachusetts. They should obtain the soleminzation certificate by submitting this application to the Secretary of the Commonwealth. Our office will need the original soleminzer certificate returned with the marriage certificate for recording.
  • Raffle Permit Application
    • To conduct a raffle or bazaar you will first need to file this application with our office. The fee is $20.00.
  • Vote-By-Mail Application
    • Submit this application annually to receive a mail in ballot for all elections that calendar year. You may mail your application, drop it off in person, email to vote@amherstma.gov, or fax it to 413-259-2499. Applications signed with a mouse, stylus, or finger are acceptable; typed signatures are not.
